 Fall Semester - August 15 to December 15

Spring Semester - January 15 to May 15 

Summer Semester - May 15 to August 15

When completing the next page you must provide your physical street address (building name/unit number, etc) under "Shipping Information" on the order form. PO Box numbers are not accepted under "Shipping Information" and will delay your order.
